Structured assessments
A qualified social worker will assess anyone referred to us. They will actively involve the service user along with their family and friends in this assessment. Once the results are in, we’ll tell you which of our services best meets the individual’s needs.

People are welcome to visit our services so that they can make an informed decision about which service to choose. We’ll then make an offer of a care package along with a hard copy of the assessment and a detailed breakdown of the financial cost of using the service (the Care Funding Calculator). We’ll never recommend a service if we don’t think it’ll meet an individual’s needs.

Induction
Once an offer of placement has been accepted, we’ll work with the individual to plan their move into Clearwater Care. This gives them a chance to get to know us and for us to get to know them. They can visit us again and take part in activities as well as stay with us overnight. This way, they can get used to their new surroundings.

Transition plan
During the transition period, Clearwater staff will work with the individual to create a support plan for the first six months. The plan will set out measurable and achievable goals for the individual including things like learning how to use a washing machine or how to make a cup of tea.

Continuous review
We’ll hold a review during the first twelve weeks of the placement to see whether the support plan is working for the individual. Further reviews can be carried out whenever the service user, their family or funding authority want.

High quality care is the foundation from which we grow from – pure and simple. That’s why we’re always looking to work in partnership with local authorities to develop our services. We sit down with local authority commissioners and operational care management teams to work out what they need from us and how we can meet their needs.
