Getting to know you

It’s important to know exactly what service users at Clearwater Care like, don’t like, are able to do and need support to achieve so that they get the best service possible. That’s why Clearwater Care’s Supported Living Team work with individuals and their support networks to understand the person’s strengths, wishes and dreams.

We will work with each person to develop a support plan that reflects what they want to achieve and how they want to be supported. This gives them the motivation to make the most of their abilities and the opportunities available in their communities.

We’ll work closely with families, advocates and other professionals so that the service we design meets the individual’s needs. We’ll work closely with individuals and their support networks to put a plan in place that makes the move into our supported living service as smooth as possible.
